Please do not use the below form if you have an urgent medical problem or you need to reschedule an existing appointment. Instead, contact the Korunda Medical Institute office at (239) 431-6464.

After you submit your request, our appointment scheduler will respond within 48 business hours.

Once you have made your initial appointment, we recommend that you complete the new patient forms prior to your arrival. You will receive a text message a few days before your appointment. Please make sure forms are filled out prior to your appointment time. As always, you can call our office with any questions or if you need help accessing the patient portal.
